All University students have access to medical care free of charge under the guidance of the University Medical Officer (UMO). The University Medical Center is further strengthened with a qualified nurse and a pharmacist. All medical reports of students should be certified by the UMO, before being submitted to the Faculty Board and Senate for approval.
Obtaining and Submitting Medical Certificates
- If a student is unable to attend to his academic activities due to illness, he/she has to submit a medical certificate to the relevant faculty. (During exam time the medical to the department of examination )
- It can be obtained from the UMO or outside doctor.
- But, only doctors registered in SLMC and registered ayurvedic practitioners can issue a medical certificate.
- The medical certificate has to be obtained at the time of consultation, not later.
Following details have to be included in the medical certificate.(please refer to the image)
- Name of the student
- Diagnosis (Illness)
- No. of days recommended for leave
- Dates
- Name of the Doctor
- Qualification
- Registration number
- Signature of the doctor
- Date of issue
